Enhanced Family Care / Shared Living

Home Care Provider

Home Care Providers play an important role in providing a safe, supportive, and nurturing home environment for individuals receiving Enhanced Family Care/Shared Living services. The Home Care Provider works in partnership with the individual, family/guardian, Hardev Services Inc. staff, and other members of the individual's support team to promote independence, health, safety, dignity, and quality of life.

Responsibilities include
  • Provide a safe, welcoming, and supportive family-style home environment.
  • Support the individual's preferences, choices, goals, and daily routines.
  • Assist with activities of daily living and independent living skills.
  • Support medication routines and health-related needs as authorized and required.
  • Assist with meal planning and preparation.
  • Support transportation and participation in community activities.
  • Promote meaningful community integration and social connections.
  • Implement the individual's person-centered service plan.
  • Provide behavioral and emotional support.
  • Maintain appropriate documentation and communicate important information to the support team.
  • Participate in required training, meetings, and quality-assurance activities.
  • Promote the individual's rights, dignity, privacy, and independence.

Direct Support Professional (DSP)

Direct Support Professionals are at the heart of the services we provide. DSPs work directly with individuals to help them develop skills, participate in their communities, achieve personal goals, and enjoy greater independence.

Responsibilities include
  • Provide individualized, person-centered support.
  • Assist individuals with activities of daily living.
  • Support personal choice, independence, and self-advocacy.
  • Assist with development of life skills.
  • Support community participation and meaningful activities.
  • Assist with social, recreational, educational, and volunteer opportunities.
  • Provide transportation assistance when authorized.
  • Support health and safety needs.
  • Follow individual service plans and organizational policies.
  • Provide behavioral and emotional support consistent with approved plans.
  • Maintain accurate and timely documentation.
  • Communicate changes, concerns, and accomplishments to supervisors and the individual's support team.
  • Protect individual rights, privacy, dignity, and confidentiality.
  • Complete required training and maintain required certifications and competencies.

Supervision Designee

Respite Provider

Respite Providers, serving as Supervision Designees, provide temporary support to individuals while helping Home Care Providers and families receive needed relief and maintain a healthy, sustainable support environment. Respite support may be provided in the individual's home or in approved community-based settings, consistent with the individual's service plan and applicable requirements.

Responsibilities include
  • Provide safe and respectful temporary support.
  • Follow the individual's person-centered service plan and applicable instructions.
  • Assist with daily routines and activities.
  • Support meaningful activities and community participation.
  • Assist with meals and household routines as appropriate.
  • Support health and safety needs.
  • Provide behavioral and emotional support consistent with approved plans.
  • Maintain appropriate professional boundaries.
  • Protect individual rights, privacy, dignity, and confidentiality.
  • Document services accurately and in a timely manner.
  • Communicate concerns and significant changes to the appropriate supervisor/Home Care Provider.
  • Complete required training and demonstrate competency.
  • Follow Hardev Services Inc. policies and applicable DHHS/He-M requirements.
